Applying for Minor Children Age 17 and Under

Everyone applying for a passport MUST appear in person, including infants and children.

For passport purposes, there are two categories of "minors":

For minor children age 15 and under - You will need to complete the Application for U.S. Passport (Form DS-11), bring the child's certified birth certificate, and BOTH birth parents must appear together with the child and provide proof of their identity – a valid driver’s license, U.S. Passport, U.S. Naturalization Certificate or government/military I.D. w/photo and signature.  If both parents are not available, one parent must appear and submit the non-appearing parent’s notarized Statement of Consent or Special Circumstances (Form DS-3053) and a photocopy of the non-appearing parent's identification.  The form is available in the City Clerk’s Office, or you can use the “Application Wizard” (see Related Links to the right).  One parent can submit primary evidence of sole authority to apply, such as a certified court order granting sole custody to the applying parent (unless the child’s travel is restricted by that order).  If submitting a court document, it must be the original document, which will be returned to you along with the passport.

For minor children age 16 and 17 - You will need to complete the Application for U.S. Passport (Form DS-11), bring the child's certified birth certificate, and it is preferred that at least one parent appear with the applying child age 16 and 17.  Many times minors in this age group may have only a learner’s permit or school I.D.  Their parent can provide his/her proper I.D. and sign along with the minor.
